This helmet plaque was part of a series of NFL/AFL plaques issued in the late 60's and sold at Sears Department Stores. It's of the Atlanta falcons and features a plastic half helmet mounted on a plastic frame. It measures 16.5" x 16.5". The frame has some scuffing and there is a crack on the lower left hand border that cannot be seen when displayed. The name plate is still intact and the back label is also present.
